What the Job Involves
You will be working within the Preparing for Adulthood Pathway as part of the Integrated Learning Disabilities Service (ILDS) team. You will work closely with other social work practitioners and health disciplines to deliver quality services. Your role involves supporting residents aged 18-25, focusing on their transition to adulthood, promoting independence, and enhancing their wellbeing. You will contribute to the pathway that focuses on preparing young residents for adulthood, ensuring the delivery of strength-based and trauma-informed services tailored to individual needs.
