You are reliable and capable of swiftly addressing urgent matters, offering both preventative and direct support.

What the Job Involves

This full-time position requires you to work 36 hours per week for an initial period of 3 months. As a Duty Social Worker within the Integrated Learning Disability Team, you will:

• Record actions and decisions clearly in the Mosaic system.
• Collaborate within a multi-agency team for health and social care services delivery.
• Provide direct advice and preventative support to adults with learning disabilities.
• Respond effectively to urgent matters to ensure the safety and well-being of service users.
• Conduct assessments and visits in emergency situations.
• Act as the first point of contact for safeguarding concerns and ensure appropriate follow-up.
• Adhere to risk management and apply principles of the MCA Act where necessary.
• Maintain high professional standards of social care practice, including quality decision-making and adherence to Social Work England standards.
