Who You Are
We are seeking a skilled and proactive Occupational Therapist to join our Adult Social Care team. This role requires someone dedicated to enabling independence and safety through major housing adaptations, manual handling assessments, and the provision of equipment and minor adaptations. You will work closely with service users, carers, and partner agencies to deliver practical solutions that transform lives.
What the Job Involves
The job involves completing comprehensive functional assessments in line with the Care Act, participating in duty triaging and screening incoming referrals, and designing major adaptations such as level-access showers, stairlifts, and ramps to promote independence and reduce risks. You will be responsible for prescribing and arranging equipment to meet individual needs, undertaking manual handling risk assessments, and liaising with contractors, housing teams, and grants officers. The role also includes maintaining accurate records and producing professional reports for funding applications and safeguarding processes. You will deliver high-quality, person-centered interventions, ensure compliance with legislation and safety standards, contribute to service development, and work effectively within a multi-disciplinary team.
Skills
- HCPC registered Occupational Therapist
- Experience in community-based social care settings
- Knowledge of housing adaptations and equipment provision
- Understanding of manual handling assessment
- Familiarity with Disabled Facilities Grants (DFG) and adaptation legislation
- Experience with contractors and housing teams
- Ability to use electronic case management systems
