Who You Are

You are a qualified social worker dedicated to promoting positive change in people's lives. You possess a strong understanding of social work methods and the legal and policy frameworks guiding adult social care delivery. You are compassionate, attentive, and committed to ensuring the well-being and independence of individuals with care and support needs while managing risks of abuse and neglect.

What the Job Involves

  • Manage a caseload of individuals with care and support needs, carrying out assessments and planning their care.
  • Engage in adult safeguarding enquiries and develop appropriate safeguarding plans.
  • Conduct financial assessments and provide information regarding services, costs, and welfare rights.
  • Plan and coordinate local authority service provisions or necessary adaptations for clients.
  • Review and reassess care and support plans to ensure continued relevance and effectiveness.
  • Collaborate with other professionals and agencies to address complex and challenging cases effectively.
  • Attend court proceedings and make recommendations for legal action as needed.
  • Maintain up-to-date, accurate records and reports for performance monitoring and quality assurance.
  • Stay informed about relevant laws, regulations, and best practices in social work.
  • Work flexibly and collaboratively to support team and organizational goals.

Skills

  • Knowledge of the Care Act 2014 and relevant social work theories
  • Experience in conducting assessments, developing care plans, and managing complex cases
  • Strong communication and interpersonal skills
  • Ability to apply the Mental Capacity Act 2005 effectively
  • Proven ability to work collaboratively across disciplines
  • Evidence-informed decision-making skills
  • Capability in managing multiple tasks and priorities
  • Commitment to professional development and ethical practice
  • Proficiency in record-keeping and report writing
