You are compassionate, attentive, and committed to ensuring the well-being and independence of individuals with care and support needs while managing risks of abuse and neglect.
What the Job Involves
- Manage a caseload of individuals with care and support needs, carrying out assessments and planning their care.
- Engage in adult safeguarding enquiries and develop appropriate safeguarding plans.
- Conduct financial assessments and provide information regarding services, costs, and welfare rights.
- Plan and coordinate local authority service provisions or necessary adaptations for clients.
- Review and reassess care and support plans to ensure continued relevance and effectiveness.
- Collaborate with other professionals and agencies to address complex and challenging cases effectively.
- Attend court proceedings and make recommendations for legal action as needed.
- Maintain up-to-date, accurate records and reports for performance monitoring and quality assurance.
- Stay informed about relevant laws, regulations, and best practices in social work.
- Work flexibly and collaboratively to support team and organizational goals.
Skills
- Knowledge of the Care Act 2014 and relevant social work theories
- Experience in conducting assessments, developing care plans, and managing complex cases
- Strong communication and interpersonal skills
- Ability to apply the Mental Capacity Act 2005 effectively
- Proven ability to work collaboratively across disciplines
- Evidence-informed decision-making skills
- Capability in managing multiple tasks and priorities
- Commitment to professional development and ethical practice
- Proficiency in record-keeping and report writing
